How do I prepare for my first visit?

You must present your insurance card and picture identification (ID) at each clinic visit. If another healthcare provider has suggested a consultation, you must ensure they have sent us a written request by the time you arrive for your first appointment. Depending on your insurance, this request may be sent to us through your insurance company.
For your first visit, bring a note from your doctor stating why you have been referred to our clinic.

If our clinic does not accept your health insurance plan, you are still welcome to make an appointment, but we ask that you pay at the time of the visit. We accept VISA, MasterCard, Discover and cash in exact amount. Our front office can tell you the cost of visit before your appointment and contact information for our billing office if necessary. You will of course receive a receipt to assist you in the reimbursement process set by your insurance plan. Similarly, patients from other states or countries are expected to pay at the time of their visit. We will provide a receipt that can be submitted to your insurance provider at home. If you are unsure whether your visit will be covered, please call the telephone number on the back of your insurance card.

We recommend that you write down any issues important to you before your visit. It will help us assist you in prioritizing your issues and deciding how many we can cover during your visit. We endeavor to treat issues important to you in a caring and comprehensive fashion, but we can’t guarantee that all issues can be addressed in the allotted time. We may suggest that you book a second appointment if more time is needed. Thank you for your understanding.
